Abstract

The Study is about effect of structured self care education for ostomate. This study is for ostomate to improve their self care during hospitalization and to promote their self care knowledge, skill and psychosocial adjustment after discharge.
The purpose of the study is to find out the different effects between the past ostomate and how well the ostomates are adjustment to psychosocial after the structured education. The structured education program was developed with literature reviews and chart reveiws of 79 cases of simple Mile¢¥s operated patients. The hypothesis is that the structured educated experiment group will be more effective on self care than the unstructured educated experiment group and that the structured educated experiment group will be more effective on psychosocial adjustment than the unstructured educated group. The design of the study was selected from ¡®S¡¯ university hospital located in Seoul, 18 people who had Mile¢¥s operation, 10 for experiment group and 8 for unexperiment group. The method of analyzing the self care ostomate effectiveness and psychosocial adjustment are using the SPSS window & Wilicoxon-rank Surnp test. The result of the study is as follow, the structured program is consisted 4 steps. The first step which is educated of third day after the operation, is application of emptying, and purchases of instruments. The second step educated on fourth day after the operation, is education of diet, control odor and management of gas. The third step educated on fifth day after operation, taking skin care of ostomate area and modeling. The fouth step on the sixth day of operation is educated on normal life, such as bathing, going out, etc. and to manage with emergency situation. This program was taken 3 hours totally, and was formed by explantation, demonstration, obserbation and modeling. The first hypothesis, which is the sturctured education group will be more effective on self care than the unstructured education group, is supported. However, the second hypothesis which is the structured education experiment group will be more effective on psychosocial adjustment than the unstructured educated group is turned down, because the uneducated group from the past was not that different from the educated group. In conclusion, even though these was no significant changes in psychosocial adjustment, the structured educated people showed increasing in self care knowledge skill, and psychosocial adjustment. Therefore education is needed continually and the patients need to be looked on the effects.
